Membership Pricing

0-18 years old $26 per month with enrolled parent
0-18 years old $42 per month without enrolled parent
19-24 years old with enrolled parent $42 per month
19-44 years old $85 per month
45-64 years old $100 per month

65 and older $115 per month
 

• Family discount (4 or more family members in same household): 10% off the total combined age-based rates. 

• For your convenience, patients may avoid a surcharge by paying via bank draft.

• A 2.8% surcharge will be applied to credit and debit card payments.
